Slim CD Case

Hello.

I have read other posts but can't seem to find an answer to my query which is . . .

How do I make a cover for the slim CD jewel case?

I've looked through the templates and can't find one to suit.
I've tried editing a couple of existing templated but don't seem to get anywhere with it.
Some help would be great!
